Eligibility & Insurance
The Health Care Center does bill and accept most insurance plans for services rendered at the Health Care Center. We do ask that students present an insurance card at the time of service and provide our clinic with the subscriber’s date of birth. (Please note that any service rendered off-campus is the sole responsibility of the student/or parent.)
Lake Superior State University has created a student health plan for our insured as well as un-insured students who maintain three or more credit hours. This student health plan covers services for our un-insured students as well as co-pays, deductibles and non-covered services of an insurance company. Well physicals and immunizations are excluded from this plan, but are offered at a reduced rate to the student. The student health plan has been established so students do not have an out-of-pocket expense. To utilize the student health plan services must be rendered at the Health Care Center.
Students who do not have health insurance may contact the Health Care Center to discuss insurance plan options. One option available for purchase is an accident and sickness plan through Student Assurance Services (this is separate from the on campus student health plan.) Lake Superior State University does not endorse this plan but simply makes it available to the student. All students from countries other than the United States and Canada are required to carry health insurance as a condition of enrollment. Students must furnish proof they have purchased an equivalent insurance plan that will cover their health care while in the United States.
